Study on the Innovative Application of Smart Elderly Care Community and Service Model

Abstract: Objective: To construct smart elderly care community and service model innovation strategy, and evaluate the effect of its
application in the management of elderly chronic disease patients in a community in Fujian Province. Methods: A total of 1 356
elderly patients with chronic diseases in a community in Fujian Province from January to December 2023 were selected as the
observation group, and 1 474 elderly patients with chronic diseases in a nearby community were selected as the control group. The
management of the observation group adopted the smart elderly care community and service model innovation strategy, and the control
group received the conventional community management of chronic diseases. The effect of blood pressure, blood glucose and blood
lipids control effect and attainment rate changes were compared, as well as changes in lifestyle, disease awareness rate, management
satisfaction degree. Results: After one year′s intervention, the systolic blood pressure, diastolic blood pressure, fasting plasma glucose,
2 hour postprandial plasma glucose, triglyceride, total cholesterol, low-density lipoprotein cholesterol, smoking rate and drinking rate
in the observation group were significantly lower than those in the control group (P<0.05). The high-density lipoprotein cholesterol,
the control rates of blood pressure, blood glucose and blood lipids, regular exercise rate, the awareness rates of hypertension, diabetes
and hyperlipidemia, and management satisfaction degree in the observation group were significantly higher than those in the control
group (P<0.05). Conclusion: The smart elderly care community and innovative strategy of service model can effectively improve the
management level and attainment rate of patients′ blood pressure, blood glucose and blood lipids, and help patients maintain good
living habits, improve the awareness rate of chronic diseases and management satisfaction degree.
